While you can create forms in HTML or Acrobat, you need some sort of CGI program on the server to accept the data the user submits and do something useful with it. That might mean emailing it to you as is, validating it, saving it to a database or just about anything else a computer can do with data.

Your ISP may provide CGI programs and documentation for common functions like accepting form input and emailing it. You may need their permission to install CGI programs that they haven't themselves supplied.
